Curtis Michael Crook

Jun 20, 1971 — Sep 23, 2026

The world lost a bit of its brightness on September 23, 2026, when Curtis Michael Crook of Indialantic, Florida at age 55 went to be with the Lord. Born on June 20, 1971, Curtis was a devoted follower of Christ, a loving husband to Heidi Lane Crook and an adoring father to Christian Michael Crook, age 8. Nothing mattered more to Curtis than the happiness and wellbeing of his family. Heidi and Christian were the center of his world. Curtis was also survived by his father Curtis Lester Crook and wife Michelle of Murietta, California, Margaret Elizabeth Walmsley and husband Harold of Peoria, Arizona and stepmother Debra Gayle Younis and husband Phillip of Parker, Arizona.

Curtis is survived by four half-brothers, Travis Crook and wife Jennie of Lake Havasu, Arizona, Ryan Crook and wife Erika of Parker, Arizona, Evan Crook and wife Samantha of Ramona, California and Matt Houser and wife Michelle of Placentia, California. Curtis also had a loving circle of aunts, uncles, cousins, nieces and nephews he was very close to.

Curtis graduated from The University of Maryland at College Park with a Bachelor of Science degree in Kinesiology in 1999 and a master’s in physical therapy from Western University of Health Science in Pomona, California in 2002. He also completed a post graduate Orthopedic Sports Medicine Residency in 2003. Curtis was TPI certified through the Titleist Performance Institute and was a credentialed Clinical Instructor with the American Physical Therapy Association.

Curtis was a compassionate, nurturing and empathetic person placing the health and welfare of others before his own self-interest. Curtis was the Director of Rancho Physical Therapy in Murietta, California where he ran a patient clinic and was involved in motocross, supercross and high school athletic teams.

Curtis moved to Florida to work with the LPGA and became co-owner of Top Tier Sports Medicine in Indialantic, Florida. Curtis provided medical coverage to the 2016 Summer Olympics in Rio and served as Physical Therapist to the USA Solheim Cup teams. In addition, he provided physical therapy support to the band Korn.

Curtis’ personality brought warmth and joy to everyone he encountered, and his passing will be mourned deeply by everyone who knew and loved him. Curtis’ kindhearted spirit will remain in our hearts forever
