She was born May 13, 1926 in Wheelwright, KY to the late Steve and Ettie Ritchie Blackburn. She is survived by her daughters, Doris Terrell, Debbie (Jim) Brown, Brenda Cummins, Sue (Ken) Witten, son, William E. Cook (Dot),a sister, Jessie McGohon, half sister, Pat Maupin and half brother, Gary Wayne Blackburn, 14 grandchildren, 30 great grandchildren and 6 great great grandchildren and several nieces and nephews. She was preceded in death by her husband, William C. Cook, sister, Dorothy Clark, brothers, Arnold Blackburn, Steve Blackburn and Gordon Blackburn. Funeral services will be Tuesday December 30 2014 at 11am at Clark Legacy Center, Versailles Road conducted by Rev. David Smith. Burial will follow in Sunset Memorial Gardens. Visitation will be from 4 until 8 on Monday at the funeral home.
