Patricia "Pat" J. Kubisiak

Patricia “Pat” Kubisiak passed away peacefully with her loved ones by her side in Fort Atkinson, Wisconsin on Tuesday, October 28, at the age of 79.

Pat was born in 1946 in Chicago, Illinois to the late John & Shirley Segal. She grew up in Chicago Heights and later moved to Wisconsin Rapids where she attended both Lincoln & Assumption High Schools. She attended UW Eau Claire and Barat College in Lake Forest, Il where she graduated MagnaCum Laude with a Bachelor of Arts in Management & Business Marketing. She went on to work for Sara Lee Corporation for 22 years. Throughout her retirement years, she worked at Fort Memorial Hospital as Manager of the Partners Gift Shop and later moved to Food Service. Pat volunteered at the hospital for many years, accumulating over 4,000 hours of service.

A compassionate and caring mother, grandma, and grammy, Pat was an avid baker, sewer, quilter, crocheter and much more.

Pat is survived by her children, Michelle Cassels and Tiffany Williams. She was the proud Grandma of Montana (Travis) Oakason, Holly Cassels, and Logan Kubisiak, as well as a loving Great Grammy to Heidi Oakason. She is also survived by her brother John (Sandy) Segal, sisters Anne Willard and Audery (Nick) Couse, brother-in-law Steve Liakos in addition to many nieces, nephews and friends. She was predeceased by her parents, John & Shirley Segal; her sister, Marsha Liakos; brother-in-law, Chris Willard, and her fur babies Rufus, Harry, and Gracie Jane.

The family would like to extend a special thank you to the staff of UW Cancer Center in Johnson Creek and Rainbow Hospice for their compassion & dedication to Pat and her family.

A Celebration of Life will be held at a later date.
