Dee Ann Nicholson-Perkins passed away in her home surrounded by family on November 29, 2005.  She was born and raised in Tempe, Arizona.  She lived life to the fullest and touched many lives along the way.  She is survived by her loving husband Michael Perkins, her parents Galen and Mary Crumbaker, sisters Vivien O'Neill and Jo Crumbaker, brothers John and David Crumbaker and 8 nieces and nephews. Memorial contributions may be made in memory of Dee to the Susan G. Komen Foundation and the American Cancer Society.
