Adam Wade Chism

Adam Wade Chism, 40, of Polk City, Florida, passed away on October 19, 2026, in Lakeland, Florida. He was born on May 22, 1986, in Plantation, Florida, to Tommy Sr. and Angela Jennings Chism. Adam worked as a concrete pump and forklift operator for 10 plus years. He was a member of the Winter Haven Moose Lodge and was of the Christian faith.

Adam was a full-time dad who loved being involved in every part of his son Jaxon’s life. They spent their time together doing just about everything, and Adam enjoyed teaching Jaxon how to play baseball and basketball. He was a fan of the Florida Gators and loved watching football. He also enjoyed trying new recipes, taking pictures, and sending TikTok videos to his sister, Amanda, and his close friend, Chris. Adam loved spending time with Chris and listening to sad country music, especially songs by the greatest, Keith Whitley. Adam was known as a jokester with a quick sense of humor who could always bring laughter to those around him.

Adam was preceded in death by his father, Tommy Chism Sr. He leaves behind to cherish his memory his loving mother, Angela (Brian) Chism; his beloved son, Jaxon Thomas (Krista Greene, Jaxon’s mom) Chism; his sister, Amanda (Ron Montilliano) Chism; his brothers, Tommy (Virginia) Chism Jr. and Jeffrey Scott (Stephanie) Chism; his brother in heart and in life, Chris (Alex) Edwards; his nieces and nephews, Noah, Rhiannon, Lindsey, Mikey, Austin, Tiffany, Cassidy, and Riley; many great-nieces and great-nephews; and many dear friends who will miss him.

A visitation will be held on Friday, October 30th, 2026 at 12:00 pm followed by a funeral service at 1:00 pm all at NorthRidge Church in Haines City, FL. His final resting place will be at Forest Hill Cemetery in Haines City, FL.
